What do I need to bring in to process a passport?

Schedule a passport appointment

To obtain a U.S. Passport you must provide:

  • Proof of Citizenship (certified birth certificate, original naturalization certificate, Expired U.S. Passport, etc.) Plus 1 photocopy of the front and back
  • Valid picture I.D. (Driver's License or State I.D.) Plus 1 photocopy of the front and back
  •  Application Form DS-11, regardless of age (bring the completed application to the appointment, use black ink or type)
  • Both parents of the applicant(s) who are under 16 have to be present. If one parent is not present please complete this form: Parent not in Attendance Form DS-3053 , or Special Family Circumstances Form DS-5525 for other circumstances.
